grade 7/8 immunizations

 
Consent forms for hepatitis B, meningococcal disease and Human papillomavirus (HPV) immunizations been sent home with all students in Grade 7 at the beginning of September. Grade 8 girls will receive consent forms for HPV immunization.
Please return the completed consent form(s) to your child’s teacher within one week, even if your child will not be receiving the immunization(s).
The meningococcal immunization or a valid exemption from the immunization is a requirement to attend school for students 12 to 17 years of age under the Immunization of School Pupils Act (ISPA).
KFL&A Public Health strongly recommends these immunizations for all eligible students.
Elementary school students must have an up-to-date immunization record or exemption from immunization on file at KFL&A Public Health. Parents must report to KFL&A Public Health each time their child receives an immunization.
The Immunization Team reviews the immunization records of all students on a yearly basis. Students who do not have an up-to-date immunization record on file at KFL&A Public Health may be temporarily suspended from school.
Booster immunizations are due between 4 and 6 years of age and again between 14 and 16 years of age. Remember to report these immunizations to KFL&A Public Health!
